Membership Selection Instructions

Please select the Curling Membership box and choose your membership type from the drop-down menu. Then select the leagues which you would like to join.

Fixed Team Leagues

Please enter the names of your teammates (if known). Each team member must submit their own registration form. Not on a team? Write TBA in the other positions, and if there is space, you will be allocated to a team.

Rotating Team Leagues

Please ensure you write your preferred positions in the notes section.

Membership Selections (no leagues)

	Social Membership $60.00 plus tax
	Spare Only and Practice Ice Membership - $260.00 plus tax

Additional Options

All adult members (18+) must select the CurlOn fee box.

If you do not wish to volunteer during the season, please select the volunteer opt-out fee.

Only check the name tag box if you need a replacement. All new members are automatically provided a name tag.

Lockers are allocated on a first-come, first-served basis.

	Mandatory CurlOn Fee $5.00. Youth Programs are exempt.
	Volunteer Opt-out Fee Full Season $200 + tax. Note: Not required for Youth and Adult Learn to Curl members.
	Volunteer Opt-out Fee Half Season $100 + tax. Note: Not required for Youth and Learn to Curl Members.
	Locker $25.00 plus tax
	Name Tag (No Charge for New Members) $15 plus tax.

Discount Options

	New Member Discount - Full Season $50 including tax (Note: Adult Learn to Curl is not eligible for this discount) This discount can be combined with the Intermediate Age Discount.
	New Member Discount - Half Season $30 including tax. (Note: Adult Learn to Curl members are not eligible for this discount.) This discount can be combined with the Intermediate Age Discount.
	Intermediate Age Discount (18-35 years of age) Full Season $50 including tax. (Note: Adult Learn to Curl not eligible for this discount) This can be combined with the new member discount. Note: Discount may be subject ot verificationn for eligibility.
	Intermediate Age Discount (18-35 years of age) - Half Season $30 including tax. Note: Learn to Curl members are eligible for this discount. This discount can be combined with the New Member Discount.
